Application for the Grant of a Patent in India
Form 1 is the request that starts a patent application in India and records who is applying and on what basis.
Form 2Provisional or Complete Specification
Form 2 is the cover sheet that carries the written description of the invention, either provisional or complete.
Form 3Statement and Undertaking About Corresponding Foreign Applications
Form 3 tells the Indian Patent Office about applications filed abroad for the same invention and promises to keep that information current.
Form 4Request for Extension of Time or Condonation of Delay
Form 4 asks the Controller for more time to do something, or to excuse a delay that has already happened.
Form 5Declaration as to Inventorship
Form 5 formally declares who the true and first inventors of the invention are.
Form 6Claim or Request Regarding a Change in the Applicant
Form 6 asks the Controller to substitute or add an applicant for a pending patent application after a transfer of rights.

Form 9 asks the Patent Office to publish an application sooner than the ordinary 18-month point.
Form 13Application for Amendment of an Application, Specification or Document
Form 13 asks the Controller for permission to amend an application, a specification or a related document.
Form 14Notice of Opposition to Amendment, Restoration, Surrender or Compulsory Licence
Form 14 lets an interested person object when a patentee or applicant asks for an amendment, restoration, surrender or compulsory licence.
Form 15Application for Restoration of a Lapsed Patent
Form 15 asks the Controller to restore a patent that ceased because a renewal fee was not paid in time.
Form 16Application to Register Title or an Interest in a Patent
Form 16 asks the Controller to enter an assignment, licence or other interest in a granted patent on the register.
Form 17Application for a Compulsory Licence
Form 17 asks the Controller to grant a licence under someone else's patent without the patentee's agreement.

Form 18 asks the Patent Office to examine an application, without which the application is never examined at all.

Form 22 is the application by which a qualified person asks to be registered as a patent agent in India.

Form 25 asks the Controller for written permission before a resident of India files a patent application abroad.

Form 26 is the document by which an applicant authorises a patent agent to act on their behalf before the Patent Office.
